Target Audience:
Physicians, nurse practitioners and physician assistants specializing in internal medicine, family practice, cardiology and cardiovascular surgery. Registered nurses, pharmacists and others with interest in advanced heart failure management are also welcome and encouraged to attend.
 
Accreditation:

Lehigh Valley Hospital and Health Network is accredited by the Pennsylvania Medical Society to provide continuing medical education for physicians. Lehigh Valley Hospital and Health Network designates this educational activity for a maximum of 5 AMA PRA Category 1 Credit(s) TM. Physicians should only claim credit commensurate with the extent of their participation in the educational activity.
 
This program anticipates being approved for 5 AOA Category 1-A CME credit hours pending approval by the AOA CCME. Sign-in and full day attendance of program is required for credit. All faculty participating in continuing medical education programs sponsored by PCOM are expected to disclose to the program audience whether they do or do not have any real or apparent conflict(s) of interest or other relationships related to the content of their presentation(s).
 
Lehigh Valley Hospital and Health Network is an approved provider of continuing nursing education by the PA State Nurses Association, an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation. Registered Nurses attending the entire program and completing the evaluation tool will receive 5.0 Contact Hours.
 
Approval of this continuing nursing education activity refers to recognition of the educational activity only and does not imply ANCC Commission on Accreditation or PA Nurses approval or endorsement of any product.
 
This program is approved for 6.8 hours of credit (.68 CEU) by Lehigh Valley Hospital, who is approved by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. A statement of CE credit will be mailed within 4 weeks of successful completion of the evaluation of the program. ACPE Program 545-000-08-008-L01-P.
